• Fax a copy of your application, program or income eligibility and proof of identity to 1-504-274-1607.
• Mail a copy of your application, program or income eligibility and proof of identity to us at 9918 Brookline Drive, Houston, TX 77099*.
*Please allow at least 5 business days for applications that are sent to us through the mail to be processed.
